What Are Health-Related Social Needs (HRSN)?
Health-related social needs are social and economic needs that individuals experience that affect their health and well-being. We help community members facing challenges like finding food, safe housing, transportation, and access to healthcare by connecting them with services and benefits that support their health and well-being.
Through community-based care coordination, our team helps connect people to senior support services, community health programs & community resources for seniors, and other local services that promote independence and better health. We can match you with a staff member and guide you through the resource connection process to meet those needs. Let’s work together to cross those tasks off your to-do list!
Eligibility Requirements for HRSN Support
We can assist you if:
- You live in King County (even without a fixed address)
- You are in need of social support services that affect your health (i.e., lack of transportation resources, limited access to food, difficulty accessing care, or other factors that may impact your overall well-being).
How Our HRSN Referral Process Works
1.Submit a Referral
Submit a referral online by clicking “Intake Form” or scanning the QR code below. Follow the instructions to sign up and complete the form. Be sure to select Sound Generations on question 1. You can self-refer or refer someone else.
2. Referral Processing
After HealthierHere processes your referral, we will receive your information and assign a staff member to assist you who will contact you shortly via phone and/or email.
3. Assessing Needs and Connecting with Resources
We will work with you to complete paperwork, assess your needs, set goals, and connect you with resources and referrals to meet your HRSN.
Frequently Asked Questions (FAQs)
Can you assist me with housing and food?
We don’t provide direct housing or food assistance at this time, but we can help connect you to available resources in King County and submit referrals to relevant programs. Please note that we cannot guarantee any specific outcomes as those will depend on resource eligibility criteria and its availability.
Why was I assigned to a different organization?
If you don’t select Sound Generations on question 1 in the intake form, you may be assigned to one of the 15 participating organizations. If you are referring someone else and filling out the intake form on their behalf, please be sure to select Sound Generations on question 1.
Do you provide services in person?
Our Health-Related Social Needs (HRSN) services are primarily offered virtually to make it easier for clients across King County to access support. We can also mail forms, resource guides, and other materials if needed to help you complete the process from home. In-person visits are occasionally available, but depend on staff availability, client’s location, and capacity.
Why does the consent form have HealthierHere’s name and logo?
HealthierHere funds our program and has developed standardized forms used by all participating organizations.
