I agree that:

1. Sound Generations and/or outside producers has authorization to use my name and take photographs, videotape, and/or record me and use, reuse, and license the use of any such photographs, videotape, and recordings, as well as my name and biography when relevant. This use is extended to the purpose of marketing materials to promote Sound Generations programs and to recruit new volunteers for our programs – both internal and external. These may be featured on program brochures, press releases, online blogs or newsletters, or other printed or electronic media.

I understand that no one is obligated to pay compensation to me or to use me in any production and that no use will give me any ownership rights. I release Sound Generations, the production team and any known media now or later from any claims or liability in connection with any appearance by me.

2. I understand that I cannot revoke my consent.

3. If I am less than eighteen years old, I will obtain the consenting signature of a parent or guardian below.

I am the parent or legal guardian of the minor named above, and I consent, on behalf of the minor and of myself, to the terms of this release.
