Geriatric Regional Assessment Team (GRAT)
Professional Referral Page
We are a team of behavioral health professionals who provide in-home assessments to connect older adults to health care and support services through referrals. Our assessment balances rapport building and learning what matters to the referred person with an evaluation of their cognition, mood, everyday functioning, and home safety. If an older adult is willing to engage in services, we then collaborate with them and their support network to develop and implement a plan that will connect them with needed services and resources. By providing outreach, assessment, and early intervention, GRAT reduces the likelihood of older adults experiencing acute behavioral health crises and endeavors to help them navigate current challenges to age in place.
GRAT does NOT provide emergency or acute crisis services.
GRAT does NOT provide case management services.

GRAT does NOT provide emergency or acute crisis services.
If an older adult is experiencing a life-threatening situation, please seek assistance at the nearest Emergency Department, or call 9-1-1.

GRAT is funded by the King County Veterans, Seniors & Human Services Levy and the MIDD Behavioral Health Sales Tax.
