Adults may face a variety of challenges as they age, specifically when they experience life or health transitions and corresponding changes in mental health, substance use, and cognition. These challenges are often exacerbated by isolation from support and resources, leading to potential crisis and increased use of emergency services.  

The Geriatric Regional Assessment Team (GRAT), a program of Sound Generations, employs a team of behavioral health experts who interface with adults aged 55+ living in King County. We reach out to aging adults at their homes and strive to complete a comprehensive biopsychosocial and cognitive assessment that can lead to risk intervention and connection to needed services and resources. By engaging aging adults as they escalate toward a crisis, our team acts to provide early intervention within a short timeframe that can reduce the likelihood of acute behavioral health crisis and the need for emergency services. 

GRAT does NOT provide emergency or crisis services.  
GRAT does NOT provide case management services.  

We honor the autonomy and dignity of aging adults. While we always strive to create rapport and engage a referred person, if they clearly refuse our assistance, we will not force participation in our program. At best, we can try to offer options to them and guidance to any existing sources of support.  

For additional information about basic program eligibility requirements, please read through our FAQ section below before beginning a referral for the first time. While we want to help as many people as possible, we unfortunately do have some restrictions on eligibility due to our funding guidelines.
